• Austin County EMS Daily Observation Report

  • This form evaluates new employees during orientation to establish a baseline standpoint. It is completed by the Field Training Officer at Austin County Emergency Medical Service. This evaluation assures a standard and quality patient care is delivered to the citizens and public that this service provides for.

  • Evaluate the employee in each of the following areas on a scale of 0 - 5:

    • 0 - Indicates the employee lacks the knowledge required to understand or function in this area. (SUBSTANDARD)
    • 1 - Indicates the employee has some knowledge required to understand or function in this area, (SUBSTANDARD)
    • 2 - Indicates the employee’s knowledge is average in this particular area. (STANDARD)
    • 3 - Indicates the employee has above average knowledge required to understand and function in this area.
    • 4 - Indicates the employee has excellent knowledge required to understanding and function in this area.
    • 5 - Exceptional

    Evaluator must comment on any score of 0 – 1

  • Evaluate physical appearance, uniform, timeliness, and readiness. Guidelines to be provided by agency specific SOG/policies.

  • Evaluate ability of trainee to pass on pertinent information about medic unit.

  • Evaluate trainee’s ability to thoroughly check/test equipment and supplies in the medic unit.

  • Employee demonstrated safe and courteous emergency vehicle operations. Always takes time to drive and become familiar with vehicle prior to an emergency situation.

  • Evaluate trainee’s ability to operate and troubleshoot all medical equipment.

  • Evaluate trainee’s ability to maintain a positive and receptive attitude towards constructive criticism and their ability to address any issues that may arise throughout the shift.

  • Evaluate trainee on protocol knowledge verbally or in written form.

  • Trainee has highly insightful, working knowledge of the medical protocol and procedures. Able to anticipate needs well in advance. Would be considered highly unlikely to have any protocols violations.

  • Evaluate trainee’s situational and scene awareness throughout call.

  • Evaluate trainee’s bedside manner and empathy for patient.

  • Evaluate trainee’s ability to complete a thorough and complete physical exam while correcting life threatening problems as needed

  • Evaluate the trainee’s ability to manage the aspects of a scene including delegation of tasks related to patient care/patient care priorities, identifies appropriate patient disposition, and patient movement to the ambulance.

  • Trainee demonstrates proper public relations with citizens, county officials, and other department staff (i.e. Hospitals, Nursing Homes, Family, Fire/FRO, PD, DPS, etc.)

  • Evaluate trainee’s ability to rapidly adapt to changing circumstances and accomplish tasks while involved in high stress situations.

  • Evaluate trainee’s willingness to identify and perform daily and shift change chores without prompting by the crew.

  • Trainee solicits criticism in order to identify deficiency then seeks to independently correct deficiencies. Accepts constructive criticism well by supervisors or other field staff.
