Third-party records requests (from attorneys, insurance companies, mediators, etc.) for copies of Medical and Billing records can be obtained from the Administrative Office by mailing the request, subpoena, and/or affidavits, along with a HIPAA release and payment to:

Medical and Billing Records are $25 for each record requested. Affidavits are $50 for each affidavit requested. Once payment is received, requested records and/or affidavits will be processed in 5-7 business days.
